Aviation Accident Investigator

Real-Life Activities

Real-Life Communication

"I cannot overemphasise the need for effective communications," says George Carney. He is an air accident investigator for the British army. "In order for the investigation to be completed swiftly, I need to communicate with many agencies that provide assistance to me. This has to be done at specific times, clearly and succinctly."

You are an aviation accident investigator and you've just written a report about post-impact fires (fires that start after a crash) in small aircraft accidents. An important part of your job is reducing risk. You want to make some recommendations to reduce the risk of post-impact fires.

Here are the facts:

  • Post-impact fire is a major cause of injuries and death in small aircraft accidents
  • Without post-impact fire, people could survive the accidents
  • There have been 521 accidents with post-impact fire
  • In 128 of the 521 accidents, fire or smoke inhalation contributed to the cause of death or serious injury
  • The benefits of post-impact-fire-resistant fuel system technology have been proven in land vehicle and helicopter applications

There are four conditions that cause post-impact fires:

  • An ignition source is close to a combustible material, such as fuel
  • A combustible material is close to the occupants
  • Occupants were not able to exit the aircraft
  • The fire was not put out in time to prevent fire-related injuries or fatalities

You must analyze the facts of this report and make safety recommendations to reduce the risk of post-impact fires.
